"Where do I find K O?"

You'll find it here, or here, or here, or here, to name a few sources.

"Do I also need to use an Ace side count when using Hi Lo or K O?"

No. Forget about ace side-counts. Nobody does that any more.

"Are there all separate indexes for K O?"

There are only a few. You'll love how simple it is. Many card counters who use more complicated systems are in denial that K-O could be so powerful because it's so simple, but K-O proves itself in simulations. I've tested it myself extensively with SBA and it's covered in Don Schlesinger's Blackjack Attack (also available here).

"How do the Illustrious 18 fit in with all these indexes?"

They fit in quite nicely. That issue was taken into consideration when K-O was developed. I think the book Knock-Out Blackjack would be a very worthwhile investment for you. It has lots of good info even if you don't adopt the system.
